Checklist for Parents and Teachers to Identify SI Problems:
___Clumsy
___Trips and falls easily
___Walks on toes or with slightly unusual posture
___Runs rather than walks, bumps into things
___Wrap feet around legs of chairs/rocks or shifts

___Frequently lays on desk when working
___Presses hard when writing with pencil/pen tasks
___Awkward pencil grasp
___Tires or frustrates easily with paper/pencil tasks
___Slow in everything he does
___Doesn't finish assignments
___Sloppy appearance and work habits
___Seems to learn something but then forgets
___Can't copy from the chalkboard to paper
___Aggressive/in fights often
___Complains about other children touching him
___Hits or kicks another child for no apparent reason
___Happest when playing alone
___Easily frustrated
___Easily distracted by noise
___Seems to avoid using the right or left side
___Difficulty or hesitancy playing outdoor games on the
playground
___Seems to know what he wants but can't "Get it together"
___Seems to ignore half of the page; reading, math...etc.
___Easily looses place while reading
___Poor awareness of direction up, down, right, left, etc.
___Hasn't established hand preference
___Reverses numbers and letters when writing or doing math
___Reverses letters, words or phrases when reading
___Skips words while reading
___Frequently turns and figits in chair
___Poor balance
___Can't seem to focus attention on task at hand
___Tires easily
___Has difficulty following verbal directions
___Poor organization of tasks
___Has difficulty remembering things presented visually
___Sees and draws things disconnected
